What is the pack?

• A review of the current best evidence or good
  practice on the subject area
• The synthesis of this evidence into a practical
  toolkit for various healthcare professionals
• This will be used to support this group of
  patients and their families
• An example of how it has been implemented in
  Durham

How do you use it?

• The hard copy / file is for reference only
• The CDrom has electronic copies of all the
  documents
• Some fields on the CD are set because if they
  were altered it would step away too far from the
  essence of the Macmillan Durham Pack
• Other fields allow you to change the content to
  reflect local practices and systems as well as
  addition of the trust logo for incorporation into
  current documentation.

What do you do with the pack ?

• The conference is only the launch
• If you take a pack you will be expected to lead
  on its implementation for your organisation
• We will be writing to each Cancer network or
  equivalent in the UK asking how they would like
  to roll it out in their area
• The network will be informed who in their area
  has a pack and asked who else needs one

Then what?

• Each network can implement the roll out in which
  ever way they feel will best work in their
  systems and structures locally
• We have a limited number of packs in hard form
• The pack will be available to anyone in
  electronic format.
