What are We Striving to Achieve

• Measurable improvements to the health, well being
  and experience of cancer survivors and their
  families
• Delivery of safe, quality, effective, and value
  for money services
• Improve communication and coordination of service
  delivery across multiple organisations and
  agencies
• Inform the cross cutting themes of information,
  commissioning and workforce

How do We Hope to Achieve This

• Develop and test approaches to assessment and
  care planning to individualise care and support
  to patients and their families
• Develop and test services that promote health and
  wellbeing
• We recognise that one size does not fit all but
  we should be able to develop a framework of
  guiding principles that will be applicable to all
  settings with local variation in application

What we are asking from Test Communities (1)

• Project plan for each of their projects
• Monthly reporting on National measures
• Completion of online learning diary
• Sharing the learning both positive and negative
• Actively contributing to the discussion board(s)
  as appropriate

• To support the development and testing of a
  framework for Assessment and care planning
• To support the development and testing of a
  treatment record
• A framework for primary care recall is currently
  being developed and will require testing later in
  your projects
