Incident Report Process

• Coach reports incident to sport coordinator or
  parish/school AD
• Coordinator/AD reports to CYO Commissioner or
  Clint Richardson
• Commissioner/Clint reports to sport commission,
  Tauno, and AAC

Incident Report Process

• Issues not dealt with at lower level first, or
  without an incident report will be sent back to
  originator.
• Resolution will be communicated to the ADs
  involved for sharing with persons involved.
