• Patient multiple providers with little shared
  information

  poor access, poor security
• Healthcare system no national standards,
  systems at high risk during organisational change

What is Informing Healthcare?

• Supporting the Patient an INDIVIDUAL HEALTH
  RECORD information to the patient and those who
  look after them
• Supporting the Clinician and Carer a number
  of SERVICE IMPROVEMENT PROJECTS clinicians,
  carers and patients with the tools, skills and
  knowledge to adopt best practice.
• Supporting the Healthcare System an
  integrated NATIONAL TECHNICAL NETWORK enabling
  information to be shared irrespective of
  organisational boundaries.
