Leading up to the Incident
  • Construction crew was on site preparing for
    planned shut in of facility.
  • Operations asked to borrow constructions Shell
    approved crane operator.
  • Job was permitted and JSEA was attached.
  • Operations personnel and the crane operator
    reviewed the JSEA prior to beginning work.

The Tasks
  • Pull the tubing bundle from the
  • WHRU Heat Exchanger
  • Inspect
  • Reinstall

Crane was boomed down at this Radius in order to

Overview of Facts
  • Crane was positioned by radio directions from
    personnel at WHRU exchanger. (unable to see boom)
  • Crane operator had very limited view of boom at
    coolers.
  • Bundle was guided out and load was taken by
    crane.
  • Bundle was inspected and reinstalled.
  • Upon laying boom back in the rest crane operator
    immediately noticed bent section and reported it
    to the facility PIC.

Incident Investigation
  • Permit and JSEA written and approved for job,
    JSEA should have been more detailed for crane
    operations.
  • Hazard of cooler proximity to boom has existed
    since the C-900 Compressor Package was installed
    and never properly addressed.
  • Additional personnel should have been assigned to
    the task during crane operations.

Safety Measures Enacted as a result of this
Incident
  • Spotter to be positioned below coolers with radio
    communication when accessing South West side of
    Deck 1 or heliport.
  • JSEAs will include detailed steps when crane
    lifts are made in close proximity to Living
    Quarters and Compressor Coolers.
  • Shell Crane Group and Civil to investigate the
    option of relocating the crane for safer
    operations.
  • Looking at additional physical barriers that can
    be installed at coolers and or boom for further
    protection.
