Health Status 1
  • Indicator implementation of personalised care
    plans and the care planning approach by
    participating in training putting in place care
    plans for a age of patients with diabetes

Health Status 2
  • Indicator review of patients attending A and E
    during practice opening hours for avoidable
    reasons.
  • Action to advise patients which services were
    available that they may have accessed instead.

Innovation and Learning 1
  • Indicator - practice attends consortium plenary
    meetings or if practice stand alone, attends
    alternative events to improve knowledge of PBC

Innovation and Learning 2
  • Indicator - practice participates in peer review
    of referrals and provides learning outcomes to
    the PCT

  • Indicator Practice has their own PBC plan (2
    points) which is based on the plan of their
    consortium reflects their health needs in line
    with the JSNA
  • Or practice provides written feedback on an
    agreed template about the plan produced by their

The Money
  • Achievement is based on individual practice
  • Each indicator carries 2 points for stretch
    achievement 1 point for standard achievement
    for the full year
  • Value of points varies by indicator depending on
    the level of work required - between 7p and 16p
    per registered patient per point
  • Maximum number of points available is 24 if full
    achievement of all targets. This equates to 1.98
    per registered patient

  • Wide consultation with practices (commenced
    November 2008 so scheme signed off and out to
    practices by 20th March 2009)

    achieve something

  • Includes identified priorities from Vital Signs
  • Supports learning and development
  • Offers some local determination
