After School Tutoring Program
Parent Permission Slip
I give permission for my child to participate in
the Green Lake School District After School
Tutoring Program. I understand that I am
responsible for the transportation, of my child,
home after the tutoring session. Tutoring will
begin at 320 pm in the library and finish at
415 pm. Tutoring will be held Monday through
Thursday. Students are expected to follow
regular school rules during the time of tutoring.
