4What Our Goals Were
- To develop a model of a rural health systems
organization for persons with stroke and their
families that can be utilized in rural
communities in Canada and internationally. - To address an identified need for community-based
rehabilitation services in southwestern Nova
Scotia. - To provide innovative and challenging learning
opportunities for students in research and
practice in an interdisciplinary, rural context.
5What We Said We Would Do
- To conduct a systematic assessment of best
practices for rural community rehabilitation
services. - To develop a profile of the target populations
and services in the Yarmouth area and identify
opportunities for student training and volunteer
development. - To share findings from the data collection phase
and design a community-based rehabilitation
program.
6What We Said We Would Do
- To develop the pilot program in the community.
- To implement and evaluate the pilot program.
- To disseminate project findings and develop
strategies to influence policy. - To evaluate the process and outcomes of the CAHR.

District Health Authority, we will spend 500,000
to pilot new efforts to prevent stroke and
improve acute care, emergency care, and
