MEMORANDUM OF AGREEMENT KNOW ALL MEN BY THESE
PRESENTS This Memorandum of Agreement entered
into in Davao City, Philippines, by and
between The DEPARTMENT OF HEALTH CENTER FOR
HEALTH DEVELOPMENT-DAVAO REGION (DOH-CHD-DR), a
government instrumentality duly created and
existing under the laws of the Republic of the
Philippines, located along J. P. Laurel Avenue,
Bajada, Davao City, represented in this act by
the Regional Director, PAULYN JEAN B.
ROSELL-UBIAL, MD, MPH, CESO IV, hereinafter
referred to as the DOH-CHD-DR
-and- The PHILIPPINE
HOSPITAL ASSOCIATION (PHA) Southern Mindanao
Chapter duly created and existing under the laws
of the Republic of the Philippines, with
principal address at Calinan General Hospital,
Davao City, represented by DR. RUBEN G. ROBILLO
in his capacity as the President, hereinafter
referred to as the PHA -and- PHILIPPINE
OBSTETRICAL GYNECOLOGICAL SOCIETY (POGS)
Southern Mindanao Chapter, duly created and
existing under the laws of the Republic of the
Philippines, with principal address at Davao
City, represented by DR. MARIBINA T. LIM, in her
capacity as the President, hereinafter referred
to as the POGS. W I T N E S S E T H

• NOW, THEREFORE, for and in consideration of the
  foregoing premises and covenants hereunder set
  forth, the parties agreed, as they do hereby
  agree as follows, to wit
• That all hospitals, public and private within the
  jurisdiction of DOH-CHD-DR shall give the
  necessary treatment or attend to a pregnant woman
  with complications from admission to discharge.
  She should be given the complete treatment
  protocol for the her case and she be discharged
  accordingly when well
• That the paying capacity of the pregnant women
  and her family shall be assessed by the Social
  Welfare Division of the hospital and if after
  proper assessment, the family of the pregnant
  women is unable to pay the bills incurred during
  the hospitalization, said bills shall be charged
  to DOH-CHD-DR, provided, however, that room
  rates, laboratory tests, medicines, blood and
  other supplies, including procedures and
  professional fees of doctors should be at the
  least possible cost
• Hospital should send the bill to the DOH-CHD-DR
  at end of each month for payment processing
• That the DOH-CHD-DR shall ensure that the
  amount received by the patient is intended to
  Indigent and strictly for the purpose for which
  it was intended.
• That all funds involved or utilized under this
  agreement shall be subject to government
  accounting and auditing rules and regulations
