event outcome for the RIEP stating that this is

32Implementing New Forms
- Per the posting on ODEs edresourcesohio.org
website, The new revised IEP form may be used
when writing IEPs in spring 2009, but it is
required to be used for IEPs written beginning in
the fall 2009 for the 2009-2010 year. - IEPs written on the current forms in spring 2009
for the 2009-2010 school year do not have to be
revised (on the new forms) in September 2009. - The new forms are posted but please note that the
parts of the annotations are still being revised.

- Reference our current forms as old and
2009 2010 forms as new forms. - New forms will be in place at the start of the
09-10 school year. - New forms are reorganization of information, some
new sections, removal of old sections. - Conversion to new forms require the old forms
be finalized, except current Progress Reports. - Old Progress Reports must also be finalized
- Any old unfinalized progress reports will be
rolled into ONE Open Progress Report Task.

- Will the new forms be automated Portrait or
Landscape? - Portrait, as they were released by the ODE
- How will expanding fields be addressed in the
Task Module? - There will be a character limit. ( There is also
a character limit on the ODE ones as well) - Trying to keep each form to a page wherever
possible - Making the fields as large as possible to avoid
attachment pages. - Will Create from Existing work on the new forms?
- Create from Existing will only work on new form
to new form.

- How will the task Model incorporate the task
status checkboxes and the Yes/No questions. If
Yes more questions area? - Software developers are still researching what
can be done to integrate these functionalities
with the Task Module. At the very least, all the
questions will be on the form with a clear
indication on whether questions should be omitted
based on previous responses. - If the IEP is done in the Spring and an Addendum
needs to be added in the Fall, does the entire
IEP need to be re-written on the new forms? - Beginning in the Fall of 2009 forward, new /
revised IEPs are required to be written on the
new forms.

- Will there be a Preschool ETR?
- The only difference between the preschool ETR and
a school-age ETR is the Evaluation Planning Plan
and not sure how this will be incorporated yet. - How many contacts can be entered?
- The Cover Page will have a place for two
contacts. - The primary contact will automatically pull in
from DASL. - The second contact will be a manual entry.
- There is an enhancement request into DASL for a
second contact record to pull over.

- Will the Goals Pages Auto number?
- The software developers are currently researching
this issue. Currently the ODE Forms do not
auto-number correctly. - How will the user keep track of Amendments to the
IEP on the Cover Page once it is finalized? - The user will create a new task, copying the
entire task from previous and then they can
update the Amendments section along with the
corresponding IEP sections. - Because a user can Create from Existing, the
entire task, it will limit the amount of work,
plus using separate tasks, keep the archive
history of the IEP clean and accurate.

- How will the user add the Visual Impairments Form
to the IEP? - The Visual Impairment Form will be able to upload
into a task as necessary. - Software Answers are also looking at specific
tasks that will include a visual impairment form
based on the disability of the student.
