    age of 85 will be increasing at a rate three to
    four times greater than the growth rate of the
    general population
  • The failure to recognize the early symptoms of
    disease, lack of frequent visits by health care
    providers, and isolation of the elderly can
    result in compounded illnesses and longer
    hospital stays, earlier admission for long-term
    health care, as well as higher recidivism rates.
  • PALS interdisciplinary program help break this

  • The goal of Partners at Living Successfully
    is to provide continuous preventative care
    coordination, combined with state of the art high
    tech health care services to help keep clients
    safe, comfortable and constantly strive for and
    maintain optimal good health of every client.
    Coordination will be made with community, state,
    federal and private resources in a comprehensive
    manner addressing all facets of a clients
    health and welfare needs.

  • Studies by the National Institute of Aging
    indicate that healthy eating, physical activity,
    mental stimulation, not smoking, active social
    engagement, moderate use of alcohol, maintaining
    a safe environment, social support, and regular
    health care are important in maintaining health
    and independence.
  • The PALS Assurance Team of professionals assists
    and advocates clients to maintain a healthy
    balance for optimal good health.

How PALS can help?
  • Over half of the senior population have
    depression issues. Depression can lead to poor
    self-care. PALS team of professionals will help
    with maintain Activities of Daily Living.
  • PALS utilizes the latest psychological assessment
    tools to help identify depression. An
    individualized plan of treatment will be
    developed in conjunction with the patients
    physician that will help alleviate depression.
  • PALS can provide seniors with the emotional
    support they need in the later stages of life.
  • PALS can also help with identifying
    emotional/mental disorders that require treatment
    or professional care
  • PALS can assess and monitor overall health and
    well-being telephonically and with a professional
    visit.

Nutritional Services
  • PALS offers a full nutritional evaluation
    initially and then every four month or more
    frequently if needed
  • PALS will help identify home meal delivery
    service and work with local food vendors as well
    as coordinate with Meals-on-Wheels and other
    programs.
  • Home health aides or participants in companion
    care can help with light grocery shopping and
    meal preparation.
  • Frail elderly in the community, in nursing homes
    or in hospitals are at increased risk of
    malnutrition. In many instances, their existing
    nutritional disorders go unrecognized and
    adversely affect their health, ability to
    overcome disease, and so is associated with poor
    clinical outcome.

Medical Equipment
  • PALS will work with local providers to ensure the
    highest quality and lowest priced medical
    equipment.
  • We will offer in home as well as telephonic
    assessments to determine when such devices are
    necessary.
  • PALS staff will be experienced in financial and
    insurance matters and provide assistance with the
    necessary paperwork.
  • Since hearing and vision decline as we age it is
    essential that our elders have the necessary
    hearing and vision care that they need. PALS can
    provide screenings, as well as references to
    local vision and hearing specialists that will
    come to the home.

  • The fastest growing segment of health care
    services is Concierge Services.bringing every
    possible convenience and service to a clients
    door step
  • We will also work in conjunction with community
    resources to assist in providing Feel Good
    services to clients, such as meal delivery, salon
    services, and companion care.

  • Evaluate the home safety environment and make
    recommendations to decrease the risk of injury
    for the elderly
  • MEDICATION MANAGEMENT
  • Offers a comprehensive report of all meds and
    their side effects. Assists seniors with avoiding
    dangerous drug interactions, wrong doses, and
    expired over the counter medications.

Financial and Legal Assistance
  • As the cost of medical and other forms of elderly
    care services continues to escalate, maximizing
    and extending client resources becomes
    imperative. Meeting these rising costs can be
    particularly burdensome for elders on fixed
    incomes, or on their caregivers financially.
  • The PALS Team of professionals can advise on
    methods to maximize client financial
    resources/benefits while minimizing costs.

Home Health Care Aides in Conjunction with
Local Agencies
PALS Assurance Team of Professionals will fully
evaluate clients initially and will periodically
stay abreast of their changing circumstances. The
PALS computer software stays current with State,
Federal and local benefits programs to help
defray costs of long-term health care.

    residence for as long as possible.

Office and Operations
  • A central office with an easy to remember toll
    free number will be staffed by social workers,
    RNs, and care coordinators. A friendly
    attentive staff will receive specialized training
    in assessing and addressing clients needs, aided
    by cutting edge computer soft ware.
  • A Comprehensive Assessment will be fed into a
    computer that will determine a clients initial,
    as well as on going needs with recommendations
    for solutions to these needs, an individualized
    life goals plan will be formulated.
  • PALS will make referrals for services to approved
    businesses and professionals that participate in
    the PALS network (through sub-contract,
    partnership or joint venture).

The PALS Smart Card
  • Clients will be given a membership card which
    will contain access to their health history
    (allergies, medications, physicians, advance
    directives, etc) and will be usable as a Discount
    Cared for many health related services
  • For Long Term Insurance clients, it will include
    the name and phone number of their Long Term Care
    Insurance carrier.

Bright Futures TechnologyPALS Partnerships
  • PALS offers a personal emergency response system
    allowing seniors to receive the fastest response
    to medical emergencies. In addition, PALS will
    provide a personal health history data service
    for clients to maximize treatment benefits, and
    minimize mistakes, poor health history on
    admission, and poly pharmacy or medication
    errors.
