  entitlements and duties. It seems to us a
  dereliction of duty to keep such a document from
  the user whose like it so directly affects

• if the care delivery system is not transparent,
  users cannot know what to expect from that
  process or what choices are open to them. Still
  less can they be expected to participate in the
  process

• People who were involved in planning their care
  were significantly happier with services than
  those who were not
• Both information and transparency in the care
  delivery process are important prerequisites for
  making empowerment possible

  relationship so that users are able to have an
