WHAT IS THE GRANT COUNTY HEALTH PLAN?

• Grant County provides for its residents
  assistance with bills for services that are
  provided by any hospital and/or ambulance
  service.

HOW IS THE GRANT COUNTY HEALTH PLAN FUNDED?
The Grant County Health Plan is funded through
gross receipts taxes and matching funds from the
federal government.

WHO CAN APPLY FOR THE GRANT COUNTY HEALTH PLAN?

• Anyone who has been a resident of Grant County 90
  days prior to the date of service from the
  hospital.

WHAT IF I HAVE INSURANCE?

• The bill must be submitted to the insurance
  provider first
• The Grant County Health Plan can cover
  insurance co-pays or remaining balances
• The Grant County Health Plan must be the payor
  of last resort

HOW DO I GET AN APPLICATION FOR THE GCHP?

• online at www.grmc.org
• any registration area of the hospital
  (including surgery and chemo)
• the business office located in the county
  building

HOW DO I APPLY FOR THE GCHP?

• Fill out an application
• Provide proof of household income
• Provide proof of residency
• Provide photo ID such as drivers license

WHAT CAN BE USED FOR PROOF OF INCOME?

• 2008 Federal or State tax return
• Social Security benefit letter
• Form stating that 2008 taxes were not filed
  and why
