VSEP Procedures Notebook Organization

First Steps

• Hold an IEP/504 meeting to determine eligibility
• Complete an Intent to Submit a Substitute
  Evaluation Form during the IEP/504 Meeting
• A separate form must be completed for each
  subject area the student is submitting a VSEP
• Obtain parent consent for photographs,
  videotapes, and audiotapes
• Create an evaluation plan
• Get appropriate signatures on both forms
• Submit both forms to Carol Jennings
• The Virginia Department of Education (VDOE) will
  approve or deny the evaluation plan within 10
  working days

• If the evaluation plan is denied, the course
  content teacher and special education teacher
  should examine the recommendations for modifying
  the plan and make appropriate changes
• The form will need to be resubmitted to the VDOE
  for approval
