  • Result of winning 2005 DOH Invest To Save bid
  • Joint Partnership Project, Bristol Adult
    Community
  • Care (ACC), Primary Care Trust (PCT), Avon
  • Wiltshire Partnership (AWP)
  • Team includes practitioners from all partner
    agencies Community Mental Health Nurse (CMHN)
  • Assessment Review Co-ordinators (ARC)
  • Community Nurses for Older People (CNOP)
  • Community Development Facilitator (CDF)
  • Based in Bristol South and Bristol North

Aims of the Service
  • To reduce the number of older people experiencing
    a crisis, avoiding inappropriate admission to
    hospital or institutional care
  • Develop a Case Management / Case findings
    approach to this preventative service
  • To increase the engagement of local communities
    in supporting their older people

  • To promote integration of health and social care
    and whole systems working, not exclusively
    focused on statutory agencies
  • Develop a more coherent strategic approach, based
    on a Community Leadership model
  • To promote the use of Assistive Technology

    prompt response (no waiting lists). Health and
    Safety a priority
  • - all users at risk of falls and who live alone
    provided with personal alarms
  • - all users / carers receive Care Repair /
    Fire Brigade environmental safety checks in homes
  • - medication alerts and specialist dispensers
    also provided

  • over 400 service users have been through the
    service
  • on evaluation score 65 showing improvements to
    independence
  • over 50 disposable alarms issued, 20 mobile
    phones, 3 charitable funding for mobility
    scooters, numerous stair lifts

Next Steps The Way Ahead!
  • Project Board, Steering Group continue to provide
    strategic leadership to the project adopting a
    whole systems approach which must include older
    people
  • Developing clear vision of what matters and what
  • counts to older people (e.g. identified need to
    set
  • up bereavement groups)
  • Further development of local services, beyond
    statutory health and social care
