Transition Overview
  • Transitions aim is to provide vulnerable young
    people 15 to 25, with physical disability,
    sensory impairment, mental illness, learning
    disability and people that are at risk of social
    exclusion with co-ordinated support to
    participate in a range of experiences such as-
  • vocational training
  • community activities, travel support and/or
  • work experience directed toward future employment

Provision
  • Referrals/Links with other services.
  • e.g. Children's services, Learning Disability and
    PDSI Adult Social Care Teams.
  • Meet and discuss Personal Plans re- Community
    based Activity/Education/Work etc.
  • Formulate Individual Personal Plan. Including
    Individual Goals Objectives incorporating
    regular reviews of the service outcomes both
    negative positive.
  • Project Support Provided
  • Individual or group staff support.
  • Travel/transport support.
  • Advice and guidance.

TRANSITION AIMS
  • The service will aim to focus on future Service
    User development regarding education, vocation
    and interaction within the community.
  • The service Aim is to develop transport training
    in order to enhance peoples social and vocational
    future opportunities.
  • The service Aim is to develop work experiences
    aimed at further skills attainment, interaction
    and vocational development.
  • The service Aims to Offer on going support within
    relevant college courses in order to promote
    individual vocational, personal and interact
    ional skills
  • The service Aims to facilitate education,
    vocational and interact ional taster sessions
    surrounding possible future individual
    development.

Transition Main Objectives
  • Service Users continue to develop their work
    experience towards Part or full time employment.
  • Service Users continue to develop their
    vocational based education and other training
    programmes.
  • Service Users continue to develop individual
    steps to overall progress and development.
