Where we were...
  • Mindset Theyll be ok.
  • Uneven Where you lived often determined what

    resulting in gaps in learning.

Registration
  • Bring your
  • 1. Your child He/she will go through a brief
    screening process while you complete paperwork.
  • 2. Birth Certificate The official certificate.
    We can no longer accept the one with the cute
    little feet.
  • 3. Immunization dates Requirements are as
    follows 4 DTP (last given after 4 yrs of age),
    3 Polio (last given after 4 yrs of age), Hep
    B(series of 3), 2 MMR (1st after 1 yr of age and
    2nd at least 28 days later, 1 dose Varicella
    (chicken pox) given after 1 yr of age or parent
    reported history of disease.
  • 4. Proof of residency PUD bill w/ address,
    Drivers license, rent receiptanything with your
    current address.

  • Remember to come to registration. Monday, May 4th
    400 700 pm in the Haller Library.
