Introducing a new Transition Tool

History of this project
  • During the summer of 2007 four states, in
    collaboration with the Technical Assistance for
    Excellence in Special Education (TAESE), met in
    Santa Fe, New Mexico to work on a product that
    would help our States move forward with
    transition planning and to improve student
    outcomes when planning for lifespan transition.
    The four States were
  • Utah New Mexico
  • Kansas Colorado

Transition Wheel
  • The Transition Wheel was created out of this
    collaboration.
  • Transition is Lifespan not just 15-21
  • Key transition points are identified and
    supported by general outcomes to assist education
    teams in planning for a lifespan of transition.

Universal Beliefs
All children should receive an education that is
rigorous, relevant, and fosters positive
relationships.

Universal goals
  • All youth will have the skills to find, attain,
    and retain meaningful, gainful, and prosperous
    employment related to their skills, aptitudes,
    and interests.
  • All youth will have the skills to successfully
    seek
  • and complete postsecondary education and/or
    training related to their interests, aptitudes,
    and skills.
  • All youth will be as personally independent as
    possible, learning to live and thrive in their
    communities as both independent contributors and
    interdependent members.

Key provisions of transition
  • A few key provisions allow individuals to
    navigate the many transition points throughout a
    lifetime (Patton Dunn, 1998).
  • Successful transitions require comprehensive
    planning, implementation of action plans, and
    coordination among all parties involved in the
    actual transition.
