Right now, two-thirds of Americans on Medicare
have more than one chronic condition, such as
diabetes, heart disease, dementia, or arthritis.
They have more doctor visits and
hospitalizations. Some say these patients will
have better, more cost-effective care when a
doctor or other qualified professional
coordinates their care. Some say Medicare should
provide an optional assessment for these patients
with chronic conditions, examining their medical
history and status, their medications, how they
function, and their need for a caregiver. There
would be a written care plan based on the
results, with chronic care coordination services
provided if necessary. They say this would
improve the quality of health care, make hospital
stays less likely, and reduce costs.
