Strategy and Diabetes Project Achievements and
Next Steps - Following information from public meeting and
care group workshops which used the ISIP
principles of transformation change to explore
the required service changes, the strategy is
now being finalised for presentation to PCT Board
at the end of September. This will be on the PCT
website in October - A PCT with local authority and GP consortium
input Commissioning Strategy and LDP workshop was
held in September, facilitated by the ISIP
consultant, to agree how the Strategy would
inform next years LDP - Within these discussions, how to move the
Diabetes project to the next stage was discussed.
This followed the draft specification being
presented to South Warwickshire PBC in August and
in September, and discussion of a Countywide
approach to Diabetes Services with Consultant
Leads and PBC leads using ABC for Diabetes
model and work undertaken in South. These
discussions were informed by the Diabetes
Commissioning Document Portfolio (Bookcase of
Diabetes) documents which had been developed. - Workshop Outcomes
- Emerging process to undertake a more inclusive
LDP process building on the successful care group
workshops and wider engagement in developing the
strategy - To complete the internal base lining exercise
linking contracts to service provision, to inform
pathway design - To develop a standard core specification for all
major contracts with identified individuals to
input and be responsible for refresh and ensure
contract compliance. This will effect and build
on the diabetes work - An emerging process for the identification of
critical service change / business cases to be
agreed. The diabetes project will be part of this
process. - .
