If you have a pre-existing condition

• This policy will only cover a pre-existing
  condition up to 2,500 for the first year that
  the person has HTH insurance. After the first
  year, HTH will cover the pre-exisiting condition
  in the same way as it would any other.

Your policy does NOT cover

• Preventative medicines, routine physical
  examines, or any other examination when you are
  not sick or injured.
• Any services or supplies not necessary for the
  treatment of sickness or injury.
• Eyeglasses, contacts, eye surgery except when
  necessary to treat an injury.
• Plastic surgery except when necessary to treat an
  injurty.

Your policy does NOT cover

• Treatment or diagnosis for infertility or
  fertility. Birth Control.
• Expenses in excess of Reasonable Expenses.
  Call insurance company to discuss expensive
  treatments.
• Injuries or sickness that are a result of being
  legally intoxicated.
• Injuries or sickness that are a result of using
  illegal drugs.

Your policy does NOT cover

• Organ or tissue transplant
• Injury or sickness that is the result of
  participating in anything illegal.
• Treatment, services, supplies or Confinement in a
  Hospital owned or operated by a national
  government or its agencies.
• The diagnosis or treatment of Congenital
  Conditions.

Your policy does NOT cover

• Expenses incurred in your home country.
• Treatment to the teeth, jaw or structures
  directly supporting the teeth, including surgical
  extractions of teeth, TMJ dysfunction or skeletal
  irregularities of one or both jaws.
• Weak, strained or flat feet, corns or calluses
• Acne and sabaceous cysts

Your policy does NOT cover

• Deviated nasal septum, unless due to an injury
• Intentionally self-inflicted injuries while sane
  or insane suicide, or any suicide attempt while
  sane or insane.
• Sickness or injury due to riot, civil commotion
  or war, declared or undeclared Loss due to
  service in the armed forces of any country.

Your policy does NOT cover

• Injury or sickness caused from riding in an
  aircraft except as a passenger on a regularly
  scheduled airline or charter flight.
• Injury or sickness caused from participation in
  professional sports, scuba diving, hang gliding,
  parachuting or bungee jumping.
• Injury or sickness from an automobile accident
  that the insured was operating without a proper
  license
