What is Healthy Rewards and how do members
access it?

• Healthy Rewards is a value-added member discount
  program, which provides access to discounts on
  services that are not usually covered by a health
  plan, but promote wellness, good health, and
  healthy behaviors
• The program provides access to services that our
  members want, at over 50,000 providers, but at
  discounted rates discounts up to 62
• To take advantage of this program, participants
  must be an NALC Health Benefit Plan participant
  and have an NALC/CIGNA ID card.
• Plan members pay for services on their own.
  Because this is a discount program, there are no
  claims forms, no physician referral requirements,
  and no limits to usage. Members simply pick a
  participating Healthy Rewards provider, present
  their CIGNA ID card and pay the reduced fee.

include coverage for any of these services, this
program is in addition to, not instead of, your
Plan Benefits. Healthy Rewards programs are
separate from your medical benefits. A discount
program is NOT insurance, and the member must pay
the entire discounted charge.

• How do members register for the fitness clubs?
• Members register directly with GlobalFit either
  online or via phone. Payment is made by credit
  card to them. Completed paperwork is faxed to the
  network club they selected within 24 hours.
• Weight Watchers?
• Healthy Rewards discounts can be used at the
  community meetings, the online subscription, and
  the at home program. Check the Franchise list to
  make sure the area is participating in the
  Healthy Rewards discount program.
• Do all Weight Watchers locations participate?
• About 75 of Weight Watchers locations
  participate. Check the list on mycarealllies.com
  to confirm.
• How do I know what the alternative medicine
  provider rates or discounts are?
• When you have located a provider on
  mycareallies.com, click on the Program
  Certificate. This shows the program fee schedule
  and discounted charges for the services. Take
  this with you to the provider appointment. Youll
  be better identified and served.

• CareAllies is
• A service mark, or brand name, used by
  International Rehabilitation Associates, Inc.
  (Intracorp). Intracorp is a wholly owned
  subsidiary of CIGNA Corporation, and a licensed
  and accredited utilization review entity.
• The PPO program available to the members of the
  NALC Health Benefit Plan includes the CareAllies
  care management and web tool services, including
  the Healthy Rewards program.
