  • You can link to all FSL forms from this website
  • Save website in favorites folder
  • Possible form updates each semester
  • Under the current members tab on the left of
    the screen click on forms/applications
  • Then look for Greek Social Event Registration
    Forms

SAFE FormSafety Action for Event Approval
  • This form is circulated around campus to UPD,
    Environmental Health and Safety, Physical Plant,
    Campus Landscape Natural Resources, the
    Facility Coordinator for the location of your
    event and the Office of Student Involvement.
  • Your organization must have a SAFE form if your
    event is on the UCF campus and
  • The event might have more than 200 people
  • Takes place in the vicinity of a pool, lake or
    pond
  • The event involves the distribution of alcohol
  • The event includes motor vehicle races, athletic
    events, marches or parades, lighting of
    fireworks, bonfires etc.
  • The completed and signed SAFE form must be
    submitted to FSL 15 days prior to your event!
  • You will also need to fill out a SERF form and
    turn into the FSL office 5 days prior to the
    event.

  • Student Organization Representative (on page 2
    and page 4!)
  • Organizational Advisor (faculty advisor or alumni
    advisor)
  • Facility Coordinator (RWC rep, building manager
    or Student Union event services)
  • Turn in to FSL Office 15 days in advance
  • Be sure to get your form date stamped and have
    FSL give you a copy for your records
  • Director of FSL will sign form and send on to
    OSI
  • You will receive a confirmation email from Sally
    Zak in OSI when your event is approved

  • Fill out on your computer so the form is legible
  • Keep your copy of turned in form on hand
  • UPD will determine if police presence is
    necessary
  • SAFE form is taken care of for all typical events
    associated with IFC Recruitment, Panhellenic
    Recruitment, Game Day Events, Greek Week Events
    Trick or Treat on Greek Street
  • Please fill out a SAFE form for philanthropy
    service events on campus, fraternity house
    parties, BBQs, when reserving space at Lake
    Claire or with Recreation Wellness Center

SERF Social Event Registration Form
  • This form is for use within FSL
  • Your organization must have a SERF form if
  • The event involves the use of alcohol
  • Grab-A-Dates, Socials, Date Functions, Formals
    and Semi-Formals, Fundraisers, Closed Socials,
    Parent and Alumni Events, Chapter or New Member
    Retreats, Capture the Flag/Manhunt, etc.
  • Your organization has submitted a SAFE form for
    the event

  • Sponsoring Organization President
  • Sponsoring Organization Social Chair
  • Sponsoring Organization Advisor (Faculty or
    Alumni Advisor)
  • All Participating Organizations Presidents
  • All Participating Organizations Social Chairs
  • Turn in to FSL Office 5 days in advance
  • Be sure to get date stamp on form and get your
    own copy
  • Your organizations GroupWise email account will
    receive a confirmation email when your event is
    approved.

SERF FormSocial Event Registration Form
  • Information due at least (1) day prior to event
    (only for events with alcohol)
  • List of sober monitors (name PID or birthdate)
  • Guest List (names and PID or birthdates)
  • For Grab-A-Dates and House Parties we must have
    list of your organization and the men and/or
    women that are your guests as well
  • Sober monitors must equal 10 of sponsoring
    organization attendees

  • Proof of Third Party Vendor
  • Call the FSL office to see if we have the liquor
    license and insurance for your venue already on
    file, 407-823-2072

  • Print out a copy of the license and bring to or
    fax to the FSL office, fax- 407-823-2929
  • Alcohol licenses that we previously had on file
    all expired on September 30, 2008.

  • Tips for SERF Form
  • Keep your copy of turned in form on hand
  • Plan Ahead!
  • A late form will result in a 50 fine for 1st
    violation, 100 for 2nd, 150 for third and
    social probation for the 4th. Make checks
    payable to GAMMA.
  • Failure to submit a form at all will result in a
    referral to the Office of Student Conduct

  • This form is for the use of Greek Council to
    create a consistent Greek Calendar without
    conflicting programming dates
  • Your organization should turn in an ARF when
  • You set a date for your philanthropy project
  • You set a date for your community service project
  • Priority is based on the date the ARF is
    submitted!

  • Turn in to the FSL Office as soon as you have
    chosen a date.
  • You will receive a confirmation email back from
    the Greek Council Secretary with your event
    reservation.
