2State Employees
- Classified employees
- are defined as permanent employees hired through
the Wisconsin Civil Service system. - are hired as Permanent, Project, or LTE

3Group Health Insurance
- You must submit an application form indicating
whether you elect or decline the insurance within
30 days of employment or within 30 days of
completion of required participation in the WRS. - Initial Enrollment
- Application must be received in the benefits
office within 30 days from your start date if you
want the earliest possible coverage. - If you elect coverage to be effective prior to
the start of the University premium contribution,
you must pay the entire premium cost until the
University contribution begins. Please refer to
page A-3 of the Its Your Choice book for the
health plans full premium amounts. - Insurance will be effective the first of the
month which occurs on or after the date the
application is received within 30 days of your
start date or the completion of the required
waiting period of state WRS service.
4Other Enrollment Opportunities
- If you decline coverage because you have health
insurance coverage elsewhere and eligibility for
that other coverage is lost or the employers
contribution ends, you may enroll in this plan
with no lapse of coverage and no penalty. - If youre appointed to a permanent position and
now qualify for the full share of employer
contributions or if your hours increase and you
qualify for a higher share of employer
contribution toward health insurance premiums. - If you get married, have a baby or adopt a child,
you will have the opportunity to enroll within 30
days of marriage or 60 days of obtaining a child.

- Premiums are paid two months in advance so you
are encouraged to submit your application as soon
as possible to avoid multiple payroll deductions.

- If you miss your enrollment period and are not
eligible for other enrollment opportunities, you
will be limited to the Standard Plan with 180
days preexisting conditions.
6Group Health Insurance Options
- You may select any health plan offered through
this health insurance program. If you select an
HMO, you must use its providers unless you have a
referral, an emergency or urgent care situation
and cannot reasonably reach a plan provider. - Preferred Provider Plans (PPP) are also offered
(including the Standard Plan). You may see any
provider but will have higher out-of-pocket costs
if you use a non-PPP provider.
7Dual-Choice Enrollment
- Each October you may elect to change to a
different health plan for the following calendar
year. - That change will go into effect January 1st of
the next year.
8Group Health Insurance Options
- Annually, the Department of Employee Trust Funds
evaluates the health plans to determine their
efficiency and quality of care, based on the age
and sex ratio of the members. - Each plan is then placed in Tier 1, 2 or 3 for
the following calendar year. Once the
Universitys premium contribution has begun, your
monthly cost is determined by the Tier in which
the plan has been placed.

10HMO Plans Limited Dental Coverage
- Some participating health plans offer limited
dental coverage, such as coverage for preventive
and diagnostic services. Some also cover a
portion of basic and restorative services. Some
plans include a limited orthodontic benefit or
discount as well. - Some health plans require that you use specific
providers who are contracted by the plan. - For specific information about the dental
benefits offered by the plans you may be
considering, see the Plan Description pages in
Section G of the Its Your Choice book.

- To find out which health plans have providers in
your area, please refer to Section A of the
current Its Your Choice book. For more
specific provider information, contact the health

Section G for information about the out-of-pocket
costs if network providers are not used.
12Standard Plan(Preferred Provider Plan)
- No dental coverage available.
- Freedom to choose physician and location of
service. - If in-network provider chosen, plan pays 100 for
benefits. - If out-of-network provider chosen, plan pays only
80 for most benefits. - There is a 100 in-network/500 out-of network
deductible per person in a calendar year maximum
of 200 in-network or 1,000 out-of-network. - See Its Your Choice booklet for additional
information. - Advantage Program requires prior notice of
non-emergency admissions, or within 48 hours
after an emergency admission. Failure to notify
will include a 100 penalty.

dental charges up to 1000 per year after a 75
per person deductible is satisfied. -

30Income Continuation Insurance
- You must submit an application indicating whether
you elect or decline the insurance within 30 days
after serving six months under the WI Retirement
System. - Coverage is effective after completing 6 months
in WRS. - Your sick leave balance determines the benefit
category for which you are eligible. - Your monthly earnings and sick leave balance
determines the monthly premium. You will begin
at premium category 1. - ICI protects your income during periods of
illness or disability by paying up to 75 of your
monthly gross income up to age 65. The maximum
monthly benefit is 4000.
31Income Continuation Insurance (ICI)
- The elimination period before benefits begin is
30 days, however, if you have more than 30 days
of sick leave, you must exhaust it (up to a
maximum of 1040 hours) before benefits begin. - There are six premium categories for classified
staff. Coverage is free once you attain category
6 (1040 hours of sick leave is accumulated). - If you earn more than 64,000 per calendar year,
you may also elect Supplemental ICI coverage to
increase your monthly benefit.

insurability - When you have initially accumulated 80 hours of
sick leave in the prior year. - The first time you become eligible for an

50Wisconsin Retirement System(WRS)
- The 2009 retirement contribution is 10.4 of most
employees gross wages and 8.5 for LTEs. The
State is currently paying the full amount for
most employees as a fringe benefit. The State
also contributes .8 to fund retiree health
insurance credits. - The earliest age to retire is 55 years old 50
years old for Protective Employees. within the
meaning of Wis. Stat. 40.02 (48), such as
police officers and firefighters. - The normal retirement age is 65 years old.
- There is no vesting requirement.
- Annual WRS statements are received in April each
year. - Prior WRS service may impact benefit application
deadlines.
51WRS Variable Participation
- There are two funds available the Core Fund and
the Variable Fund. - You are automatically enrolled in the Core Fund.
You may elect to also participate in the Variable
Fund. - You have 30 days from your start date to complete
the election form for the current year. Submit
the form directly to the Department of Employee
Trust Funds (ETF). - If you elect to participate in the variable fund,
50 of all future contributions (employee,
employer and additional) will be deposited in the
variable fund. - If you dont elect to participate in variable for
the year in which you were hired, you may elect

beneficiaries prior to your death, any State

59LEAVE BENEFITS
- Vacation, sick leave holiday allotments for
represented and non-represented classified
employees are similar, but you should refer to
your collective bargaining agreement for paid
leave information if you are a represented
employee. - Limited Term Employees (LTES) are not eligible to
accrue sick leave or earn vacation, holidays or
other leave. - You receive vacation prorated according to
anticipated hours in pay status during the
calendar year. - You may begin using vacation after you complete
six months in an original appointment. - Vacation is allocated on a calendar-year basis.
- In the event you terminate employment, the
University will recover the value of vacation
used but not yet earned.
60PERSONAL HOLIDAYS
- You receive 4 ½ days (36 hours) of personal
holiday leave per year as soon as you begin work
and you have until the end of the calendar year
to use them. - Part-time appointees receive a proportionate
share consistent with percentage of appointment. - Should you leave university employment prior to
completing six months of probationary service,
the University will recover the value of the
personal holiday used but not earned.
61LEGAL HOLIDAYS
- Full-time employees earn 8 hours of leave for
each legal holiday. Part-time employees earn a
pro-rated amount. - To be eligible to be paid for a legal holiday,
you must be in employment status on the holiday
and do at least one of the following - Work on the holiday
- Be in pay status on the last scheduled work day
immediately before the holiday or - Be in pay status on the 1st scheduled work day
immediately following the holiday.
62SICK LEAVE
- Sick leave is accrued at the rate of .0625 hour
for each hour in pay status. - A full-time employees will accumulate 5 hours of
sick leave for each 80 hours of pay or 16.25 days
per year. - If you are in pay status for less than 76 hours
per biweekly period, you will accrue a pro-rated
amount. - Sick leave may be taken only after it has been
earned. - Under rules of the State Office of Employment
Relations, employees may use sick leave for
personal illness, bodily injuries, maternity or
exposure to contagious disease for immediate
family or personal medical or dental appointments
which cannot be scheduled at times other than
during work hours for temporary emergency
medical care of ill or injured members of the
immediate family (up to 5 days), or for a death
in your immediate family. - You can not use sick leave for a snow day.

- Your unused sick leave balance is lost if you
terminate employment before retirement unless you
have at least 20 years of WRS creditable service.
The amount of sick leave that you have
accumulated will be converted at your highest
rate of pay. This can then be used to pay for
health insurance premiums in the State of WI
Group Health Insurance program after you have
applied for an annuity. - If you terminate employment and return to a
permanent position within 5 years (3 years for
terminations prior to 7/5/1998), your sick leave
will be restored. - If laid off, you may use your sick leave credits
to pay for State Group Health Insurance for up to
5 years. - At retirement, the value of accumulated sick
leave credits may be converted to pay for your
State Group health Insurance.
64Federal (FMLA) Wisconsin (WFMLA) Family
Medical Leave Act
- Job-protected leave with continued medical
benefits for when you need time off to care for
yourself or a family member who is seriously ill
or to care for a newborn or newly adopted child. - Leave taken for FMLA-eligible reasons, must run
concurrently under FMLA, WFMLA and other leave
available to university employees.
65FMLA USAGE
- Call HR if you will be off more than three
consecutive days for a serious health condition. - Serious health condition is defined as a
disabling illness, injury or impairment or a
physical or mental condition involving inpatient
care or continuing treatment or supervision by a
health care provider.
66FMLA Continued
- Generally does not apply to the flu or a common
cold, ear aches, upset stomach, minor uclers,
headaches other than migraines unless it involves
visits to a healthcare provides and/or qualifying
treatment. - On your timesheets, indicate flu, cold, etc. so I
dont call you to see if it qualifies for FMLA.
