  • Read the following pages and notice the words in
    bold-face (darkened words).
  • Each school is different, but many have the same
    rules.
  • Call or go to the school nearest you to find out
    the rules.
  • If you do not have all the information needed, go
    to the school anyway to start registration.
  • Dont let your child miss school.

When to Register Your Child
  • Register your child for Kindergarten or other
    grades before school starts.
  • Schools usually starts the last week of August to
    the week after Labor Day.
  • The child must be five years old before September
    1 of the school year.
  • Each school has its own registration rules.
  • Schools have similar requirements that need to be
    followed.

Requirements (Rules)
  • Birth Certificate
  • Social Security Number
  • Proof of Residency in the school district.
  • Physical Examination (some schools)
  • Proof of guardianship or custody (some schools)
  • Immunization Records

Birth Certificate
  • The parent or guardian should have proof of the
    age of the child (five years old before September
    1). An official birth certificate is required
    not a hospital certificate.

Social Security NumberSome Schools need this
  • The child needs a social security number not the
    adult.
  • If you child does not have a social security
    number, you can get one from the Social Security
    Office. For information

Proof of Residency
  • The parent or guardian must prove that they live
    in the area (district) of the school. To show
    that you live in the district you need
  • Mortgage papers, lease agreement and a notarized
    District Affidavit if residing with relatives.
  • Photo Identification State Driver's License or
    ID, Passport
  • Phone Bill
  • Utility Bill (gas, electric or water bill)

Proof of Custody/Guardianship
  • Some schools require that you show that the child
    is under your care. You may need one of the
    following
  • Court Order
  • Divorce Decree
  • DCFS Legal State Placement Papers

Immunizations Very, Very Important!!!
  • Immunizations or shots are necessary to enter
    school. Most states require the following shots
  • DTP (Diptheria, Pertussis, Tetanus)Series of 4
    doses
  • Polio (Trivalent, oral or inactivated) Series of
    3 doses
  • MMR (Measles, Mumps, Rubella)one dose on or
    after the first birthday
  • Hib (Haemophilus influenza type b) per
    immunization schedule
  • Varicella (Chicken Pox)one dose on or after the
    first birthday

  • If your child does not have the shots, you can
    get them for free or at low cost at clinics.
  • To find a clinic near you, call your school or
    town.
  • You can also find the information on the web by
    searching Google.
