- On September 14, 2004, at approximately 200 pm,
Mary Clark ran into Ricky Young. The accident
occurred on South College Street next to Veggies
To Go when Mary Clark fell asleep behind the
steering wheel while waiting at the red light.
The first officer to the scene of the accident
was Officer Seals. Officer Seals reported Mary
Clark was driving a green 2003 Honda Civic and
Ricky Young was driving a white 2004 Cadillac
Sedan Deville.

- I have designed an electronic Alabama Uniform
Traffic Crash Report. In order to access the
form all officers must log in. If you do not
have a user id or password you must create one
using the registration page. This feature will
make it easier for officers to access and amend
old reports. After you have created a user id
and password you will be given a list of all of
the old Crash Reports you have filled out. If
you are interested in creating a new Crash Report
you can click on the link at the bottom of the
page. The electronic Alabama Uniform Traffic
Crash Report is the same as the form that each
officer fills out by hand. This electronic
report is an attempt to make filling out the firm
user friendly and convenient.

- One 3.5-inch high-density disk drive
- A network adapter card
- A monitor supporting 800 x 600 resolution in 256
colors - Microsoft mouse or compatible pointing device
- Printer
- Internet Access with a 28.8k modem dial-up
connection - Email Access
- Fax Machine
- Windows PC (Windows 95, 98, Me, NT4, 2000, XP
Home or XP Professional) or Apple Mac. - CD ROM drive or DVD ROM drive
- 32MB RAM - preferably 64Mb RAM for better
performance

The officer will be asked to log in. If they
dont have a user id and password they will go to
the registration page. If they do have a user id
and password they will go directly to the Crash
Report Page.

6This is a database of all the crash reports that
an officer has filled out. This makes it easier
for an officer access various accidents. Some of
the attributes are Local Case Number, OPS Case
Number, Date, Driver, DOB, etc.

Crash Report. It is broken down into various
sections Location and Time, Driver, Vehicle,
and Areas Damaged.

Alabama Traffic Crash Report. This page displays
driver and vehicle information.

the Alabama Traffic Crash Report. This page
displays the Areas Damaged and a button to
continue to the next page of the Crash Report.
10This is the second page of the Alabama Traffic
Crash Report. This page displays information on
the other person involved in the accident. It
consists of Driver, Vehicle, and Areas Damaged.

information pertaining to the Vehicle and Areas
Damaged.
