Task Conduct quarterly III Corps Safety
Council Purpose To provide Corps senior
leadership a forum to review safety performance,
share lessons learned, and provide a platform for
the development of ideas through dialogue and
presentation of practical tools. End State To
provide a safe training and working environment
for the entire III Corps team.
2Motorcycle Fatality AAR 41st Fires
3Lessons Learned(Findings and Recommendations of
the Accident Review Board)
- Present and contributing The Soldier was
travelling 35mph in excess of posted speed limit. - Recommendation 1 Sustain current III Corps
and 41st Fires standards in reference to the
conduct safety briefings at Company and Battalion
level. Continue to emphasize discussion at the
1st line supervisor level. - Recommendation 2 Utilize Big Brother to
Little Brother counseling and safety brief
orientation when communicating with Soldiers
about safety. - Present and contributing The Soldier recently
upgraded from a Yamaha R6 (600cc/414Ibs
motorcycle) to a Yamaha R1 (1000cc/454Ibs
motorcycle). - Recommendation 3 Mandate that motorcycle
riders communicate to their Company/Battery/Troop
Commanders whenever they upgrade the capability
of their motorcycle. IR should trigger
additional counseling from company-level
commanders and a referral into the 41st Fires
Brigade Motorcycle refresher course (a supplement
to III Corps mandated motorcycle training). - Recommendation 4 Initiate monthly
brigade-resourced motorcycle refresher training
that consists of a motorcycle-side route brief,
safety discussion, and a mini-Phantom Thunder
Run. Voluntary for all motorcycle qualified
brigade Soldiers and mandatory for all recent III
Corps motorcycle safety graduates (facilitators),
and Soldiers who purchased new motorcycles.
4Corrective Actions Summary
- Sustain current III Corps and 41st Fires
standards in reference to the mandatory conduct
of safety briefings at company level. -
- Sustain the use of the Accident Risk Assessment
process to mitigate accident potential of high
risk Soldiers prior to release for the
weekend/leaves/passes. - Initiate brigade-resourced monthly motorcycle
refresher training to supplement III CORPS
motorcycle training program. - Mandate that motorcycle riders communicate to
their company-level commanders whenever they
upgrade the capability of their motorcycle. - Motorcycle capability upgrade triggers referral
into the 41st Fires Brigade Motorcycle refresher
course (directed in written counseling
statement). - All redeploying brigade motorcycle riders
participate in 41st Fires Brigade Motorcycle
refresher course during mandatory reintegration
training period. -

- Issue Speeding
- Discussion Driving over the posted speed limit.
It was identified as a contributing factor in
this tragic accident. - Recommendation Continue to emphasize the
consequences in driving over the speed limit.
7Lessons Learned
- Issue Seatbelts
- Discussion The passenger of the vehicle was not
wearing her seatbelt and was thrown from the
vehicle, which resulted in her death. The driver
knew that the passenger was not wearing her
seatbelt, yet continued to operate the vehicle.
This death could have been prevented if the
passenger wore her seatbelt. - Recommendation Leaders need to continue to train
and emphasize the importance of wearing
seatbelts, especially since the leading cause of
death in automobile accidents is being thrown
from the vehicle. Leaders should also stress the
importance of the battle buddy system and
watching out for each others safety.
