Complementary and Alternative Therapy (CAM)
  • 5 Domains (NCCAM National Centre for
    Complementary and Alternative Medicine)
  • Alternative (homeopathic medicine and Chinese
    medicine)
  • Mind-body interventions (visualisation and
    relaxation)
  • Manipulative and body based (chiropractic and
    massage)
  • Biologically based (vitamins and herbal products)
  • Energy therapies (qi gong and therapeutic touch)

12
Domains
  • Alternative
  • evolved separate from conventional
  • Mind-body
  • meditation, prayer, mental healing, art, music
    and colour therapy
  • Biological based
  • Herbs, foods, vitamins, sharks cartilage
  • Manipulative and body based
  • chiropractic, osteopathy
  • Energy therapies
  • Biofield - Reiki, qi gong
  • Bio electromagnetic - pulsed fields, magnets,
    current

13
How widely used
  • 1998 study showed amongst general public use had
    increased from 34 in 1990 to 42 in 1997
  • 2000 study reported 37 of 46 patients with
prostate used one or more therapies (herbal, old
time recipes, vitamins and special diets)
  • 2000 study 83 of 453 cancer patients had used at
    least one therapy (diets, psychotherapy,
spiritual practices) 69 had used at least one in
    their cancer therapy

14
Evaluation of CAMs
  • National cancer institute and NCCAM are funding a
number of clinical trials to evaluate CAM
    therapies

15
NCI and NCCAM Sponsorship
  • Either the alternative therapy in comparison to
    conventional or the complementary therapy
    alongside conventional therapy
  • Enzyme therapy with nutritional support
    inoperable pancreatic cancer
  • Shark cartilage for advanced breast cancer
  • chemotherapy plus radiotherapy with or without
    shark cartilage in NSCLC inoperable
  • studies on the effect of diet on prostate and
    breast cancer

What patients should be advised
  • Discuss with doctor - some CAMs interfere with
    conventional therapy
  • Ask
  • what benefits can be expected
  • What are the risks
  • Do the benefits outweigh the risks
  • What side effects
  • Will it interfere with conventional therapy
  • Is it part of a clinical trial if so whos the
    sponsor

18
Therapies utilised
  • Hypnosis Reflexology
  • Acupuncture Biomagnets
  • German Neural Therapy Healing/Reiki
  • Prolo/Sclerotherapy Naprapathic
  • Water therapy Relaxation
  • Herbs/Homeopathy Massage

19
Hypnosis
  • The conscious use of the natural trance state,
    linking the mind through suggestion
  • Referring to the suggestion, the acceptance
depends on the motivation of the client
  • Possible to anaesthetise by influencing the
    autonomic nervous system
  • Stammers, phobias, facial ticks, habits, acute
and chronic pain, labour pain, stress management,
    phantom limb pain, insomnia

20
Pain control - hypnosis
  • Fear
  • Unknown, unending pain, death
  • Belief
  • endorphins
  • Focus
  • On the pain and reduction
  • Relaxation
  • visualisation and guided imagery
  • Dissociation

21
Reflexology

25
Acupuncture
  • Treating pain injuries by inserting fine needles
into acupressure points on the body
  • accompanied by manual, electrical or heat
    stimulation
  • Stimulation of nerve fibres in muscle or facia
    sending impulses to spinal cord
  • Endorphins and cortisol
  • neurological and musculoskeletal pain
  • Migraine, neck pain, sciatica

26
Magnets
  • Magnets help to balance the acid-alkaline
imbalances and increase calcium ions, increase
blood and oxygen circulation and carry away
    toxins, neutralise histamine
  • Unipolar
  • either north or south, North relieve pain, reduce
    swelling, promotes tissue alkalisation, promotes
restful sleep, increases oxygenation to tissues
    and calms nerves, South Opposite
  • Bipolar

27
Water Therapy
  • Good for Fibromyalgia - chromic pain
  • Goal is to rehabilitate a specific area of the
    body
  • Reduced weight bearing
  • Reduced joint compression and support
  • Reduced effort with slow movement
  • Increased resistance

28
Others
  • German Neural therapy
  • nerve block using procaine - local anaesthetic
    for nerve ganglia, anti-inflammatory,
    re-establishes polarity
  • Prolo/sclerotherapy
  • After injury ligaments can become stretched, body
tries to stabilise causing spasm - concentrated
dextrose or calcium gluconate to cause sclerosing
of the ligaments, sets of the healing process of
    collagen
  • Naprapathic
  • Manipulation

29
More
  • Cryoanalgesia
  • cryoprobe-compressed nitrous oxide or C02
  • Epiduroscopy
  • visualisation of epidural space and perform
    injections
  • Neurolytic injections
  • Damage neural tissue
  • Radiofrequency procedures
  • Electrical current to block pain transmission
  • Spinal cord stimulation
  • Electrical stimulation of a precise area in the
spinal cord to block pain transmission
