  • 140,000 people with Developmental Disabilities
    are currently being served by OMRDD.
  • Developmental Disabilities served include Mental
    Retardation, Autism, Cerebral Palsy, Epilepsy,
    and Neurological Impairments.

  • Contact the OMRDD Developmental Disabilities
    Service Office in your area.
  • Complete an OMRDD Eligibility Transmittal Form
    with the assistance of DDSO Staff.

    with documentation of the persons Developmental
    Disability.

  • The active assistance offered persons as they
    navigate the community and various service
    systems in pursuit of the necessary and desired
    services and supports that will assist them in
    achieving or maintaining their personal goals.

  • Help to people to explore what they want and need
    in life and assisting them in getting it.

    live as independently and productively as
    possible.

  • Working with the person to arrange and conduct
    planning meetings, collecting information, and
    using the information to create a vision of needs
    and desires and what help is needed to obtain
    these valued outcomes.

    valued outcomes and locating or creating supports
    and services.
  • Upholding a persons rights and ensuring that
    the persons living arrangement is safe.
  • Writing the Individualized Service Plan record
    keeping to remain in regulatory compliance.

  • In order to receive Medicaid Service
    Coordination, a person must meet all of the
    following requirements
  • A person must have a documented diagnosis of a
    developmental disability.
  • A person must be enrolled in Medicaid.
  • A person must demonstrate a need for ongoing and
    comprehensive, rather than incidental, service
    coordination.

  • MSC requirements continued
  • A person, or an individual authorized to give
    consent on behalf of the person, must choose to
    receive MSC.
  • A person must not permanently live in an
    Intermediate Care Facility, Developmental Center,
    Psychiatric Hospital, Small Residential Unit,
    Nursing Facility or Hospital or any other
    Medicaid funded setting that provides service
    coordination.
  • A person cannot be enrolled in any other
    comprehensive Medicaid long-term service
    coordination program.

  • Ensure your childs service needs are known to
    OMRDD at least 3 years prior to graduation.
  • Early identification will enable you to work with
    OMRDD to establish services and supports
    proactively to make the transition from school to
    adult life as smooth as possible.
  • Early identification will help to ensure the
    services that you desire will be available from
    the agency of your choice.

  • DDSO staff, or your Medicaid Service Coordinator,
    can assist you to complete a needs survey for
    your child.

  • Work with agencies to determine if the programs
    they operate can meet the needs of individuals.
