2The link in the weekly reminder email will bring
you to the login page
After you return your participation form, you
will receive an email containing your username
(your email) and your password to log into the
site. You will be prompted to change your
password the first time you log in.
3After you log in for the first time, you will
visit this screen above. Complete as follows
- Enter your name
- Make sure your email address is correct
- Enter contact phone number
- Change your password
- Click save and you will see text below
4After you have updated your account information,
you are ready to navigate the system using the
center tool bar under the High School RIO icon.
- Home allows you to see your current login
information. - Admin allows ATCs to manage their account
information, student rosters, and generate
coaches reports of athlete restrictions. - Injury/Exposure Reports Dashboard is where
ATCs will enter exposure and injury data weekly - Students allows ATCs to update athletes they
are treating or input an entire team roster (can
also be visited through Admin screen). - My Account is where ATCs should frequently
check to make sure their email address is up to
date (can also be visited through Admin screen). - Log Out should be used to exit the High School
Rio site at the end of every session.
5To begin reporting study data, click on the Week
you would like to add an Exposure Report.
The system will not allow you to add an exposure
report for weeks in the future.
Make sure the week you click corresponds with the
week dates you are reporting for.
6Important definitions of injury and exposure are
provided at the beginning of the weekly exposure
report
- Definitions
- Injury A reportable injury is one that
- Occurred as a result of an organized high school
practice or competition during the past week AND - Required medical attention by a team athletic
trainer or a physician AND - Resulted in restriction of the student-athlete's
participation for one or more days beyond the day
of injury OR - Resulted in any fracture, concussion, or dental
injury regardless of whether or not it resulted
in restriction of the student-athlete's
participation. - Exposure An athlete exposure is defined as one
athlete participating in one practice or
competition where he or she is exposed to the
possibility of athletic injury. Exposure will be
expressed in two parts - Number of athlete-practices the sum of the
number of athletes at each practice during the
past week. For example, if 20 athletes practiced
on Monday through Thursday and 18 practiced on
Friday, the number of athlete-practices would
equal 98. - Number of athlete-competitions the sum of the
number of athletes competing during the past
week. For example, if 9 athletes played in a
Freshman game, 12 in a JV game, and 14 in a
Varsity game, the number of athlete-competitions
would equal 35.
7For each sport, select whether or not the sport
is in season. If the sport is in season, enter
the number of athlete-practices and
athlete-competitions that took place in the past
week. Also enter the number of injuries that
occurred.
- Once you have entered information for all sports,
click submit at the bottom of this page.
8After you have submitted your Exposure Report,
you will see either a red triangle, a yellow
triangle (above), or a green check (above) next
to the week you reported for..
Red Triangle Exposure report is 3 or more weeks
late and needs your attention.Yellow Triangle
Exposure report is due or you have submitted an
Exposure Report that may have errors.Green
Check Exposure Report has been submitted with
no errors.
9- After completing the exposure report, an injury
report form will automatically appear if you
reported any injuries on the weekly exposure
report. - Injury report forms can also be initiated
manually by clicking on Add Injury from the
Injury/Exposure Report Dashboard.
10The number of injuries reported in the weekly
exposure report should match the number of injury
report forms submitted for the week. If you
become aware of additional injuries, please
update the weekly exposure report with the new
number of injuries.
The completed Week 1 exposure report can be
modified by clicking Week 1.
11This tab has been made available to you for your
clinical use as a token of our appreciation for
your participation in the study. You do not have
to enter any data on this tab to participate in
the study other than create a new student. The
researchers will never see any of the information
you enter on this tab.
Here you have 3 options. Select a pre-populated
student name from the existing list, create a new
student, or have the system generate a
de-identified student code with no accompanying
name. You must do one of the 3 to access the rest
of the tabs.
12This is the Athlete Demographics Tab. You need
to enter data on this tab to participate in the
study to the best of your ability. If you
selected a student from a pre-populated list
some of the demographics will populate
automatically.
Boxes will turn red or you will receive an error
message at the top of the page if data is entered
in the incorrect format.
A sport must be selected in order to save your
injury report.
13This is the Injury Info tab. You need to enter
data on this tab to participate in the study.
Continue to answer the questions to the best of
your ability.
If mouth/teeth is selected, a question
regarding mouthguard usage appears below this
question.
If knee or ankle are selected, a question
regarding type of knee or ankle injury will
appear below.
14This is the Injury Event Info tab. You need to
enter data in this tab to participate in the
study. Continue to answer the questions to the
best of your ability.
When choosing Other in any of the dropdown
menus or checkboxes, a box will appear for you to
type in a description.
15This is the Sport-Specific Info tab. You need
to enter data in this tab to participate in the
study. Continue to answer questions to the best
of your ability.
Click save as you go through the report. When you
are finished, click save and exit.
16The EMR tab allows you to track an athletes
current participation status, to enter SOAP
notes, and to document assessments, treatments,
and outcomes.
This is the Medical Notes or EMR Tab . This
tab has been made available to you for your
clinical use as a token of our appreciation for
your participation in the study. You do not have
to enter any data on this tab to participate in
the study. The researchers will never see any of
the information you enter on this tab.
17EMR Tab (continued) This portion of the EMR tab
allows the ATC to document assessments ,
treatments and outcomes. A current date or date
in the past must be entered to Add New Note. As
notes are added they are displayed
chronologically with the most current on top.
Anything typed into the Medical Notes section
will be displayed if you prepare a student report
for the athlete. You do not have to use the
student report tool.
18After you have completed an injury report, click
the Save and Exit button. You will receive
this message indicating the report was saved and
you will see this box. Click yes.
19The reported injury is shown on the main page. To
modify this report, click Edit.
If the injury report form needs to be deleted
(i.e., you report an injury for a player you
thought would be out at least one day but the
athlete returned to practice the next day so they
no longer meet the definition of injury, the same
injury was reported twice, etc.) it can be
deleted by clicking X.
This generates a Student Report. The
information used to generate this report comes
from the EMR tab of the injury report.
20Clicking on the Summary tab allows ATCs access
to the Student Report page. . This has been
made available to you for your clinical use as a
token of our appreciation for your participation
in the study. You do not have to enter any data
on this page to participate in the study. The
researchers will never see any of the information
you enter on this tab. This page allows ATCs
to generate a Student Report of injury details
and recommendations. The information used to
generate this report comes from the EMR tab of
the injury report.
21You can print the Student Report by clicking
the print button and selecting a printer. You can
save the report electronically (to email to a
parent, for your files, etc.) by clicking the
print button and selecting Adobe PDF and then
saving the file.

22To add a new student click Add New Student.
Edit student information.
Delete student from manage students.
This is the Manage Students page that ATCs can
access by clicking on Admin or Students.
This has been made available to you for your
clinical use as a token of our appreciation for
your participation in the study. You do not have
to enter any data on this page to participate in
the study. The researchers will never see any of
the information you enter on this tab. This
screen provides ATCs with a convenient way to
keep track of their student athletes
information. Information entered here will
pre-populate the corresponding variables on any
injury report form completed for a student.
23You can print this report by clicking the print
button and selecting a printer. You can save the
report electronically (to email to a coach, for
your files, etc.) by clicking the print button
and selecting Adobe PDF and then saving the
file.
This is the Coaches Report page. To access this
page, ATCs can enter their Admin page and
click Generate Coaches Report. This has been
made available to you for your clinical use as a
token of our appreciation for your participation
in the study. You do not have to enter any data
on this page to participate in the study. The
researchers will never see any of the information
you enter on this tab. This page allows ATCs
to generate a report of athletes with
participation restrictions for the coach of any
sport. The information used to generate this
report comes from the EMR tab of the injury
report.
24Important Reminders
- Complete an exposure report for every week that a
sport is in season, even if your teams are
inactive (if a team is inactive, simply report
0, 0, and 0 for athlete-practices,
athlete-competitions, and number of injuries). - Weekly exposure reports and injury report forms
can be modified at any time. - If you need assistance or have any concerns, the
