3Opening Doors to the Future Accomplished
through a continuum of transition services in 7th
12th gradeTransition Scope and Sequence
focuses on acquisition of knowledge and skills to
plan and make decisions for the future.
4Knowledge and Skills to Open Doors to the Future
is provided through a Chest of Tools.This
Chest of Tools empowers students to envision
and plan for their future.

Knowledge of their Disability and how they best

Ability to function as a member of

6The Chest of tools is kept in a Vocational
Portfolio the portfolio begins with a shell of
information that the students build and
personalize, so they have resource material and
personal examples to take when they leave high
school.
7Students need to self assess their strengths,
interests, and needs using formal and informal
assessments. Formal assessments used such as
the COPS-II
8Informal Assessments completed at least on an
annual basis assessing employment, post
secondary training, recreation/leisure, daily
living, adult living skills, and self advocacy

interviewed before?What type of accommodations
do you need for post secondary training?How will
you locate a place to live?Have you ever filed

9Gathering of information and skills is
accomplished using a Transition Curriculum
Transition Curriculum provides 3 courses
Life Skills- Will be Offered in 8th grade
Employability Skills I- offered in 10th or 11th
grade Employability Skills II- offered in
11th or 12th grade

15Students review and follow transition flow charts
for each of the post secondary options.Students
use transition flow charts and teacher guidance
to develop their own plan of action.

17Community linkages are also developed through a
County Wide Transition Fair.The Fond du Lac
County TAC will host approximately 60 community
agencies and post secondary schools.Students
from Campbellsport High School along with
students from neighboring schools will be invited
to attend.
18Community linkages are stored in a community
resource guide. The guide provides agency
descriptions and contact and access information.
19Community linkages developed through internships
involve commitment between students, employers,
and the school. The internship allows
students to learn about a specific career area
and gain job related knowledge.
20Community linkages are also developed through
participation in School to Work Programs, Youth
Options, Volunteer Programs, and Teacher
Assistant programs.
