Training Course Request Form

• Guidelines for form completion
• Course request must be completed by Chief or
  Training Officer of the host jurisdiction
• Funded course requests must be completed a
  minimum of 60 day prior to start of course
• Non-funded and Reimbursable course request forms
  must be completed a minimum of 30 days prior to
  start of course

Components of the Course Request Form

• Areas of interest to be completed by Host
  Authority
• Section A of the course request form must be
  completed prior to submittal
• Side two (2), Instructor(s) names and course
  topic information must be completed with purposed
  hours to be taught.

• Place a check in the appropriate box for
• Funded
• Non-funded
• Reimbursable
• Begin form with identifying Host Agency Name

Body of the Course Request Form

• Identify County/City/Town
• Class to be taught
• Location where training will be held
• If you would like the course advertised on the
  VDFP Website
• All Funded courses will be listed

Body of form continued

• Proposed start date
• Proposed end date
• Including testing if applicable
• Number of students expected
• 12 students minimum for Funded training
• Estimated hours for training expected.

Reverse side Instructor Information

• List of participating Instructor(s) that hold
  current Train-the-Trainer
• Number of hours Instructor(s) will be teaching
• After action hours are to be completed at end of
  course
• Any hours over minimum on funded courses must be
  pre-approved by VDFP Division Chief

Accompanying paperwork

• If course has multiple modules a copy of the
  schedule listing module names, dates, locations,
  and instructors must be submitted with course
  request form to enable Division Office staff to
  process request form

• Please note when requesting HTR courses,
  requests must be submitted by June 1, for the
  fall teaching semester and December 1, for the
  spring.
• All Funded requests must comply with current VDFP
  disbursement policies
