Medical Reimbursement Dependent Care Assistance

• Current carrier, EPOCH, will continue to offer
  coverages in 2005
• 2005 Medical Reimbursement Contribution - 400
• Prorated for part-time employees
• Employees can still elect payroll reductions for
  medical reimbursement
• Available in accounts January 1, 2005

Deadline for Claims

• It is important that you notify your provider
  that your insurance carrier will be changing for
  2005.
• Claims should be filed by December 31, 2004
• If any large claims are incurred at the end of
  2004, please notify the Benefits Office at ext
  2083
