3This form is not as daunting as it may
seem.This training is designed to assist you in
understanding the CSD 295.
4Please NoteThe form is divided into two
sections for your convenience 1) data collected
on individuals 2) data collected on
families.The items collecting data on
individuals are yellow.The items collecting data
on families are blue.
5Lets get started.
- We will review each item on this form.
6Item 1 Contractor Information
- Please make sure you fill in the correct
Contract and Reporting Period
7Items 2 3 are intended to collect the number of
individuals that received services.
- ITEM 2 Collects the number of people served
that you were able to collect at least one
characteristic about. - ITEM 3- Collects the number of people served that
you were not able to collect any characteristics
about.
8Items 3 4 are intended to collect the number of
families served.
- ITEM 4 Collects the number of families served
that you were able to collect at least one
characteristic about. - ITEM 5- Collects the number of families served
that you were not able to collect any
characteristics about.
9Remember
- An individual is considered a family of one!
- This means the totals in items 4 5 include
individuals. - Item 4 should at least be equal to item 2 (and
item 5 should at least be equal to item 3)
10Avoid Duplication!
- Be sure that you are tracking services carefully.
- Be sure that an individual receiving seven
services is recorded as one (1) individual and
not seven (7) individuals.
11Item 6 - Gender
- On the new form you will not be able to save
your form if the total for Item 6 is greater that
the total reported in Item 2. - Why?
- For example, if you reported in Item 2 that you
served and collected information about 1,000
individuals it is impossible to report the
gender of 1,001 individuals. - (This is also true for item 7)
12Item 7 - Age
- Record the age of each individual receiving
services. - Note that this section is in yellow, which means
you are recording the information only for the
individual receiving service. - If an individual comes in to get services and
they tell you they have a three year old son,
unless the son is receiving services, do not
record the sons age here.
13Item 8 Ethnicity/Race
- Record Ethnicity (This total cannot exceed the
value reported in Item 2). - Record Race (This total cannot exceed the value
reported in Item 2) - Please note that you report information under
both Race and Ethnicity. - NOTE Two new categories have been added to Item
8. This year, this form will also collect Asian
and Native Americans.

15Item 9 Education Level of Adults
- Please Note
- Item 9 only collects the education level of
adults ages 24 - Therefore, the total reported in item 9 cannot
exceed the sum of 7e 7h reported in item 7
above.
16Item 10 No Health Insurance Disabled
- Item 10 a b collect two sets of information
- the number of persons reporting and the number of
people surveyed. - The number of persons reporting an answer cannot
exceed the number of person surveyed. - For example, if you surveyed 1,000 individuals
it would be impossible for 1,001 individuals to
respond.
17Item 11 Family Type
- Ask everyone, even individuals, their family
type. If they are an individual, record them
under 11d as a single person. Remember
Individuals are a family of one. - On the new form you will not be able to save
your form if the total for Item 11 is greater
that the total reported in Item 4. - Why?
- For example, if you reported in Item 4 that you
served and collected information about 500
families it is impossible to report the family
type of 501 families. - (This is also true for item 12)
18Item 12 Family Size
- Record all responses to family size under 12a.
- Remember Even a single person has a family size
of one.
19Item 13 Family Income
- Please note
- This item collects the income sources of the
entire family. - Each source of income for the family should only
be reported once. - Lets look at an example..
20Examples
- If one family receives SSI, TANF and General
Assistance mark 13c (TANF), 13d (SSI) and 13g
(GA) each only once. -
- If mom and dad both receive SSI mark 13d (SSI)
only once. - If mom has a job and dad collects a pension
mark 13f (pension) and 13i (employment other
source) each only once. - If mom and dad both have jobs mark 13j
(employment only) only once. -
21Item 13 Source of Income Continued
- Please Note If a family reports that they have
no income mark 13b. - Note that a family reporting no income is
reporting their source of income (their source is
that they have no source) - Thus this family should be included in 13a (the
number of families reporting one or more sources
of income).
22More information on Item 13 Source of Income
- Because Item 13a is asking for the number of
unduplicated families reporting their income
this number should be close to the number
reported in item 4 (total number of families you
collected on or more characteristics about). - Because items 13b 13k are collecting sources
of income for the family and are recorded only
once per family the numbers reported in each
line item (13b 13k) should not exceed the total
number reported in 13a. - 13b-k are not added.
- For example, if you collect the sources of
income for 500 families it is impossible to
report that 501 families receive SSI
23Item 14 Level of Family Income
- Record the level of family income for each
family using the Federal Poverty Guidelines. - The Guidelines are available at www.csd.ca.gov
24Item 15 - Housing
- Record the type of housing of each family
25Item 16 Other Family Characteristics
- Record each family claiming to have the
characteristics of Farmer, Migrant Farmworker
and/or Seasonal Farmworker -

CharacteristicsLets do an example!
27An African American mother (Sue) comes in with
her three children looking for services. She is
married to a Latino man (Jose), who has a full
time job at a local cannery. They live in a two
bedroom apartment in the county. The oldest
child (Jason) is 12, and is from a previous
marriage of Sue and is African American. The
other two children (Marie, Joseph) are biological
children of Sue and Jose. They both identify
with the Latino ethnicity and consider themselves
as mixed race. Sue is 38, Jose is 43, Jason is
12, Marie is 9, and Joseph is 6. Jose has his
GED and has attended some college but never
finished. Sue graduated high school. She is not
currently working, and has been unable to find
work. Sue receives food stamps and SSI. The
family income is 20,548. She is looking to get
services to help her build her skills and find
employment. She is also requesting youth
programs for her three children. The family does
not have health insurance. The agency will be
providing services to Sue, Jason, Marie and
Joseph.How do you record this information?
28Another example
29You administer a program on Sunday mornings that
passes out bagged lunches to the low-income
people in your community. The program is
administered by volunteers and they are usually
very busy passing out lunches. This particular
Sunday was especially busy. The volunteers
counted 512 people that received a lunch,
unfortunately they did not collect any additional
information on any of the people. The next
Sunday was also busy. This week, the volunteers
counted 127 people receiving lunches. One of the
volunteers thought they recognized some of the
people from the previous week, but they couldnt
be sure. How do you record this information?
