What happened. MTC-EMPLOYEE TRIPS, FALLS TO
THE GROUND AND FRACTURES FINGER Employee was
walking around front of forklift and his heel
caught the tip of the fork, causing him to trip
and fall forward. The employee extended his
arms to break his fall and consequently fractured
his left ring finger when he contacted the
ground. The forks on the forklift were flush to
the snow-packed ground and the area had adequate
lighting. The employee did not land on any
object on the ground, nor was he carrying
anything in his hands. Incident was immediately
reported to Rig Manager, Tool Pusher, HSE
Technician and Shell Foreman.

- All personnel must remain focused on recognition
and mitigation of all hazards, no matter their
perceived level of risk or frequency of
opportunity. A small lapse can have significant
consequences. - Designated a parking area for the forklift where
exposure to routine paths of travel is reduced. -
