• INCIDENT Whilst climbing a ladder the base of
  the ladder slipped causing the ladder to fall to
  a lower shelf. The injured person fell to the
  ground and was hit by a piece of equipment that
  fell from the lower shelf, resulting in a
  fracture to his pelvis.
• FAILURE
• Correct rules/procedures were not
  followed/communicated.

• LATERAL LEARNING
• Portable ladders should be secured, prior to
  their use - Ladders shall not be used unless they
  have been physically secured or they are being
  held by a second person.
• Fall Protection - Once in position the person
  working at height should secure himself to the
  structure with a suitable harness and lifeline.
• When working at heights, helmets must always be
  worn with chin-straps.
