What is my Health Reimbursement Account and how
does it work?

• Springboro Schools provides funds to all Plan
  members not currently enrolled in the HSA to help
  defray certain out-of-pocket expenses related to
  their Health Plan Deductible.
• Expenses must be incurred within the calendar
  year to be reimbursed.
• Remaining yearly Health Reimbursement Account
  Balances will be carried forward into the next
  Plan year.

How Do I Get My Money?
Complete a CLAIM FORM, These forms can be found
on our website. Complete required information
on claim form and attach an EXPLANATION OF
BENEFITS from your medical carrier showing the
amount of deductible and out-of-pocket expenses
incurred within the appropriate Plan year.
Amounts above required patient liability will be
reimbursed. All reimbursements are mailed to
the employee at their home address or direct
deposited per the participants request.
