• More accurately capture LTCOP activities
• Promote greater consistency of documentation
• Provide an easier process for documentation
• Provide a clear record of agreed upon resolutions
• Promote accuracy in coding complaints
• Create a complaint record that could potentially
  be shared with other agencies
• Enhance coordinators quality control

Feedback on New Forms

• Suggestions for Coordinators
• Hold a focus group with volunteer ombudsmen to
  introduce the form and solicit feedback
• Field test the forms
• Talk with your data entry person
• ASOs will be in contact with coordinators
• Provide comments to the State Office by January
  9, 2009
• Revised standard forms will be finalized by March
  2009
