  • IEP team must determine whether a student meets
    significant cognitive disability (SCD) definition
    as the first step of Form 1 eligibility criteria.

The 21 Rule for Eligible Students
  • 21 prior to Sept. 1, 2004 take assessment
  • 21 on Sept. 1, 2004 take assessment

    assessment ever again

Forms Availability
  • Begin now!
  • SPED Attachment C (in lieu of ADE Form 1)
  • Form 2 available on ADE web (below), TUSD will
    distribute 1 hard copy to each teacher with
    eligible students at trainings - working on an
    electronic data collection solution
  • Form 4 Parent Interview
  • Form 5 Data Sheets Instruction 4 Activities

  • Level II forms are pending state board approval

Form 4 Parent Interview
  • Grades 3, 4, 5, 6, 7, 8, 10, 11, 12
  • Parent must be given the opportunity to
    participate in interview process with a certified
    ExEd staff member (not a paraprofessional) who
    understands how to assess the information
    supplied from discussion with the parent
  • Do not send form home for parent to complete on
    their own!
  • Can be quickly done in an IEP meeting or through
    a phone call!
  • It is a parents choice whether or not to
    participate

Data Entry System
  • New! Web-based system (formerly known as AADSI)
    will incorporate the alternate assessment changes
  • Transition to ADE Common Logon system
  • Anticipated date for data entry is January 5,
    2005.
  • Collect data now dont wait!
  • Assessment aligns with IEP goals and objectives.

Current Timelines
  • Online system will be available January 5, 2005
    through March 31, 2005 for teachers
  • Heads Up! New ADE system will require teacher
    logins and passwords
  • ExEd Department will submit the information to
    obtain passwords
  • Update new Form 2 watch for new information!
  • Complete Form 4, Parent Interview (this has not
    changed for Level I)
  • Provide instruction on Forms 5 A-D (this has not
    changed for Level I)
  • Use Data Sheets
  • Attend 2 hour training when schedule becomes
    available

Data Entry System
  • So, please . . .
  • DO NOT use the old forms!
  • If teachers use the old forms, when they start
    data entry, items WILL NOT MATCH!

Mandatory Trainings
  • Due to the many changes, all ExEd teachers who
    have students who qualify for alternate
    assessments will need to attend 2 hour trainings.
  • Salary increment credit will be provided!
  • Watch your email and the web site for information
    as to dates and locations
  • Anticipated start for trainings mid Nov.
