  register on-line to access SafeGuards
  interactive web portal. The following
  information displays the steps required to
  complete the registration process to receive a
  username and password for log-in.

  requires the following elements to ensure
  security and confidentiality of members
  information
• Enrolled subscribers first and last name
• Enrolled subscribers date of birth (DOB)
• Enrolled subscribers SafeGuard family ID or
  social security number (SSN)
