Background
To address SEPCos safety performance
plateau and recurring incident findings, several
key improvement opportunities identified from the
study included were

• Improving work place hazard recognition
• Eliminating work planning/process
  inefficiencies, and
• Improving overall HSE cultural commitment.

In 2006, a Project Execution Team was charged to
develop strategies and coordinate implementation.

Changes found in the revised Standard 008

• Merging of old onshore (HSE0008B) and offshore
  (HSE0008A) Standards
• Allows for greater consistency between all
  facilities
• Excerpts from OSHA Regulations referenced as
  external documents
• Less wordy, more references
• Removed Welding Burning Procedure
• Removed Excavation Procedure
• Added new terminology to Standard more Roles
• Global change to all SEPCo facilities
• It is condensed reader friendly

  Matrix (JPM)

• Added electronic tool, ISSOW, to enhance job
  planning, documentation, work authorization, and
  knowledge transfer
• Streamlined and interconnected Work Certificates,
  Job Safety Analysis, LOTO, Confined Space Entry,
  Safe Work Plans and SIMOPs
• Name changes for SWPA roles (Performing
  Technician, Area Authority, Isolation Authority,
  Work Party, etc.)
• Color-coded permit system (stamping of permits no
  longer necessary)
• Now two types of Cold Work permits (regular and
  breaking containment) and two types of Hot Work
  permits (flame and spark potential

• Added new type of permit called Energized
  Electrical Work
• Added improvements to LOTO isolation procedures,
  including provisions for use of "lock box"
• New feature for De-isolation for test.
• Eliminated exclusive control option for
  hydrocarbon service
• Allowable entry level reduced (more conservative)
  to 20.7 to 20.9 Oxygen (from 19.8)
