GP Referral Reports - trends
  • GP Elective referrals have increased by 11 in
    the last 12 months. This report, produced after
    each month end, allows Practice to see their own
    referral rates for individual specialties.
  • Here, this Practice had a period when referral
    rates were higher than those for its neighbours.

Practice Information Dashboard
  • The new Information Dashboard will replace the
    Excel Quarterly Packs sent to Practices, and
    re-present much of the information available in
    Ardentia in a more usable way.
  • It will serve as a tool to communicate a
    Practices PBC performance, co-ordinate messages
    and actions, and act as a supported gateway to
    all available PBC information.

RISC patient stratification
  • RISC reports an identifiable list of patients at
    risk of unscheduled chronic admission for a
    Practice. (PARR is unidentifiable and needs to be
    decrypted into Excel by the PCT).
  • The clinician can quickly filter this list by
    demographics and clinical condition.

  • One click takes the clinician to all the clinical
    data (Acute and GP) concerning that patient
  • By comparison, PARR provides only an NHS number
    on a spreadsheet that the clinician must then go
    and research from other systems (Ardentia or GP
    system).

  • To improve and maintain the health of Somerset
    residents through cost effective and safe
    prescribing
  • To make best use of the prescribing budget to
    provider further freed up resources for member
    practices to invest in patient care
  • To ensure as new clinical care pathways are
    developed making best use of medicines within
    them that budgets are adjusted to reflect this.

    secondary care activity budget would allow for a
    comprehensive debate concerning increased
    prescribing reducing hospital admissions

    something been done to us, but something that we
    can do.
