- In your opinion, WHY have these incidents
happened? - At a personal level think about and discuss
- If you were to have an accident because you know
you are not following a basic safety rule, what
would that incident be? - When would you have it? How bad would it be?
- What would be the consequences for you, your
family, your team? - At a team level think about and discuss
- If an accident was to take place in your team
because you know someone is not following a basic
safety rule, what would that accident be? - When would it happen? How bad would it be?

4Some questions to consider in case you work in an
office environment
- Do you intervene when you see an unsafe act or
condition? It is your duty! - Have you recorded any near misses or incidents
using STOP cards or the Impact incident reporting
system? - Do you set aside a period during the week/month
specifically to consider or read about HSE? - Does your performance contract include HSE
elements? - Are you familiar with the HM tools have you
applied any? - Do you keep your office workplace tidy and free
of hazards? - Have you participated in an office HSE
inspection? - How do you manage your work life balance?
- How do you support interior operations so that
they have more time to manage safety? - Do you know who your first-aider and fire-warden
are? - Do you know the emergency exit.
5Suggestion for a personal or team Safety Pledge
- Suggestion If needed modify the following
statements to suit the team, - print it and sign as individual and/or a team.
- I WILL (We WILL).
- intervene when I see unsafe behavior I have a
Duty to Stop the work. - use a seatbelt and make sure all other car
occupants also wear seatbelts. - adhere to all traffic rules and never use a
mobile phone when driving. - observe and follow all HSE signs, policies and
instructions. - ensure all work instructions are understood
before starting an activity. - wear and maintain appropriate PPE and comply with
site PPE rules. - ensure that all Permit To Work requirements are
adhered to. - use the right tools in the right way for every
job. - keep work sites and office clean, tidy and
obstruction free. - always hold the handrails on stairs.

- Safety is not the first priority it is the equal
priority in everything we do. - The role of the leader in moving forward is to
achieve step change - People are natural risk takers
- People are naturally lazy
- You need to generate false rewards in safety to
motivate people - The toughest job of a leader is safety - because
it is not natural. - Accident investigation is a waste. You're not
expecting it. You're pressed for time. There is
an atmosphere of fear. Can you get to a root
cause? There are NO NEW ACCIDENTS. - People practise for their accidents. So stop them
practising! - Leaders who go out before the accident
demonstrate their interest in safety. Leaders who
go out after the accident demonstrate...? - Investigations are a sign of respect for the
people we injure or kill. - If we got better at going out the day before, we
wouldn't have to go out the day after. - Biggest problem in industry today is people - and
what are we doing? Email. - 99 of incidents are caused by people.

responsibility for an accident, the greater the
crime. - The HSE champion has to live in the top office
- Societal expectations around HSE are being
globalised through mass communication - Leaders see their own behaviours reflected in the
way their people behave. - Shouldn't the Director come to the site of the
incident, rather than demanding the site come to
them? - A leader regularly goes into the workplace to
identify weakness and compliment success - We have confused auditing with testing and
verification - auditing is a leader's job and
only he/she can do it. In our world, auditors
shoot the wounded - Documents what we have on our shelves are
promises - what we have in the workplace are
practices - Definition of safety awareness consciously
looking for risk - Procedures are usually inappropriate in style and
content. They should be written by users for
users - There is nothing predictable or common about what
sets off an accident - Reward leading HSE indicators, not lagging
- Keep up praise for success - it moves performance
on. Praise repetition. Reward the value of the
effort. Vary praise.
