• Worksite Hazards
• Background
• The fatal accident which recently occurred near
  Sahma booster station, on a pipeline construction
  site, has highlighted the need to examine hazards
  of peripheral operations. The victim was
  assisting in re-fuelling welding sets with diesel
  fuel. When walking between the slow moving diesel
  truck and the welding sets, he was run over by
  the truck. Both the victim and the diesel truck
  driver were sub-contracted. The victim was
  relatively new to the operation. The victim had
  been instructed with regard to fire hazards
  associated with the re-fuelling operation but had
  not received other instructions.
• Recommendations
• PDO and Contractor Supervisors are reminded to
• watch and examine all activities associated with
  core and peripheral works on site with a view to
  identifying potential hazards. A good approach is
  to observe and ask the question what if ...?.
  Dont assume that its safe because its always
  been done like this.
• ensure staff and workers have complete
  instructions before carrying out their jobs.
  Toolbox talks should be held be each work group,
  on site and before the start of the daily work or
  any special tasks.
• provide additional or more intensive supervision
  to new employees during their initial period of
  orientation on the work site.
• work together with their sub-contractors in
  regard to the above
• PDO Contract Holders should remind their
  Contractors that all Sub-Contractors are required
  to be approved by PDO.
• Learning Points
• Seemingly simple tasks can involve exposure of
  the worker(s) to unidentified hazards
• Long established and much repeated operations are
  not necessarily safe.
