• INCIDENT
• The Electrician (IP) was descending the stairs
  quickly, when he lost balance. He held firmly on
  to the rails to avoid a fall. This resulted in
  dislocation of his right shoulder.
• Root Causes
• Lack of Due Care Attention
• Failure to follow rules and procedures
• LEARNINGS
• Stairway should be ascended or descended one step
  at a time and by using the handrail maintain
  3-point contact at all times
• Do not rush while ascending or descending the
  stairs
