KOHP Summit - 01-25-11
  • 48 participants from Sedgwick, Harvey and Butler
    Counties and State agencies
  • Convened by KDHE KDOA
  • Purpose to join others in your community to
    shape the vision for the Self-Management Program.
    Resulted in Sedgwick County participants wishing
    to continue meeting on a monthly basis - this was
    the start of the Sedgwick County KOHP Board

Development of Sedgwick County KOHP Board
  • February 25, 2011 - First meeting
  • Discussion focused on results of the Summit
    addressing logistical barriers and challenges to
    growing KOHP in Sedgwick County
  • WSU Aging Studies and the Sedgwick County K-State
    Research and Extension Office offered leadership
    in addressing logistical challenges
  • WSU Aging Studies would provide student help
  • Sedgwick County K-State Research and Extension
    would serve as the central administrative site
    and supervise students
  • Kansas Health Foundation Recognition Grant
    application

KHF Recognition Grant
  • Purpose to develop a systematic delivery of KOHP
    to provide consistent availability of the program
    in Sedgwick County
  • Centralized program administration
  • Support for the program facilitator teams
  • Reduction in time spent by facilitators in
    administrative tasks
  • Creation of an efficient, stream-lined system
  • Consistent provision of KOHP workshops
  • Development of a participant scholarship fund

  • Develop
  • Workshop supply kit (charts, markers, name
    plates, etc)
  • System for managing logistical arrangements
  • System to provide participant snacks
  • Develop marketing and outreach plan
  • Create supports to meet course leaders needs on
    an on-going basis

  • Develop community partners across the county who
    are willing to host the program in their
    facilities
  • Develop a system for referring program
    participants through community partnerships
  • Coordinate the scheduling of the program and
    participant referrals and registrations through a
    centralized system housed within the Sedgwick
    County K-State Research and Extension Office and
    supported by Aging Studies graduate students.
