    of correction than a verbal on-the-spot
    correction

  • Used to report a significant event which may
    impact the operation or security of the facility
  • May be positive or negative reports about
    inmates, especially health and welfare of the
    inmates
  • Reports must contain as much information as
    possible about the incident
  • Forwarded to the Guard Commander

  • Used to report inmates violations of facility
    rules and/or UCMJ
  • Report must contain as much information about the
    infraction as possible to include a DD Form 2823,
    Sworn Statement, by issuing cadre member and by
    cadre witnesses to the incident
  • Forwarded to the Guard Commander
