• On-the-Spot Corrections
• minor violations of simple facility rules when
  inmate was unaware or does not repeat regularly

• Domicile Entries (DEs)
• minor infractions which require a greater degree
  of correction than a verbal on-the-spot
  correction
• maintained in the inmates living area
• Guards discretion whether a DE should be used in
  lieu of a Disciplinary Report

• Observation Report
• Used to report a significant event which may
  impact the operation or security of the facility
• May be positive or negative reports about
  inmates, especially health and welfare of the
  inmates
• Reports must contain as much information as
  possible about the incident
• Forwarded to the Guard Commander

• Disciplinary Report
• Used to report inmates violations of facility
  rules and/or UCMJ
• Report must contain as much information about the
  infraction as possible to include a DD Form 2823,
  Sworn Statement, by issuing cadre member and by
  cadre witnesses to the incident
• Forwarded to the Guard Commander

• Used in conjunction with a Disciplinary Report
• Used to record the facts of an incident
• Used to record thoughts and impressions as well.
