Handle with Care !!!!!

• What happened ?
• A small portable dry powder fire extinguisher
  exploded. Luckily nobody was hurt.
• This extinguisher had been used on a water-truck
  for about 9 months. It had failed a
• recent inspection (faulty pressure gauge) and was
  set aside under a shade for
• collection by the Maintenance Contractor.
• Why did the accident happen ?
• All the required specifications and procedures
  had been followed
• Fabricated to the right standard
• Inspected on a monthly basis
• Serviced annually (by a certified company)
• Yet it still exploded. Most likely because of
  external damage.
• It had not been handled with the necessary care.
• What can you learn from this accident ?
