MTM Consensus Definition
  • MTM is a distinct service or group of services
    that
  • optimize therapeutic outcomes for individual
  • patients. MTM services are independent of, but
    can occur in conjunction with, the provision of a
  • medication product.

Approved July 27, 2004 by AMCP, AACP, ACA, ACCP,
ASCP, APhA, ASHP, NABP, NACDS, NCPA, and the
NCSPAE.

MTM and Medicare Part D
  • MTM is a stipulation of Part D legislation
  • Part D Plans must have MTM program
  • May be provided by pharmacists

    minimum)
  • Multiple chronic conditions
  • Multiple medications
  • High drug costs (gt4,000 in 2006)
  • Plans must reimburse providers for time and
