NDC Outcomes

• Improve how people rate their own health
• Decrease the proportion of adult population who
  smoke
• Increase the uptake of physical exercise
• Reduce the number of children on the child
  protection register in the area
• Reduced Teenage pregnancies/conceptions
• Reduce the numbers of admissions to hospital
• Reduce the level of alcohol misuse in the area
• Reduce the level drug misuse in the area
• Reduce the level of mental ill health
• Increase the proportion of people who eat 5
